Outcome or output?
An output is what you deliver. An outcome is the change that happens because of it.
Keeping them separate makes plans and reports honest: activity counts show what happened, while outcome evidence shows what changed.

Digital support sessions
Fictional worked examples for learning. Organisational records need your own checked information.
- Name the activity or service that was delivered.
We delivered six introductory digital-skills sessions for 20 participants.
- Describe the change you expected for people, organisations or a place.
We wanted participants to feel more confident completing an online form without help.
- Identify a proportionate record that could show whether that change happened.
We will ask the same confidence question before the first session and after the last, report how many answered, and explain that self-reported confidence is not proof of independent ability.
